All student athletes participating in any and all sporting events must have this document signed before they are eligible to play.

Concussion Information, Knowledge and Consent form, & Post concussion consent form
This document, to be signed by both a parent and a physician, gives medical clearance for a students athlete to return to play. This is in addition to the Player Record Card which will be given to a student athlete by their coach on their first day of practice and is to be returned immediately. Failure to return all mandatory documents will result in a forfeiture of participation for the sports season.

IHSA Constitution, By-Laws & Policies
The IHSA Constitution and By-laws are the framework of rules that govern interscholastic competition among IHSA member schools. Every provision of the Constitution and every By-law was voted into effect by the membership and can be changed only by a vote of the membership. The Board of Directors approves interpretations for situations that need elaboration or are not specifically mentioned in the Constitution and By-laws.

IHSA Policies and Procedures are enacted by the Board of Directors to provide guidance for the day-to-day administration of IHSA sport and activity programs and the operation of the Association.

The IHSA Handbook is a compilation of all these items, published every summer for the coming school term.

IHSA Physical Examination form:
 
All student athletes must have a sports physical examination form on file with the school to participate in athletics. The exam is valid for a period of one year. Please bring all sports physicals the the athletic director, Michael Martinez, or place in his mailbox in the main office.
